What can we do to combat biases?

I recently came across this chart in an HBR article depicting how women are still at a significant statistical disadvantage in hiring decisions.  75% of women finalists-only 67% getting the job,  25% women finalists-0% (yes ZERO percent) getting the job...

What can fix this?  Using better interview debrief methods to assess all candidates with collective intelligence.
